Common problem among split type mortise lock.
Most budget type lever handle has this problem. One of the problem lies in the construction of the spindle. Solid or curved type spindle is not stable enough hence causing the lever to loose or just come off later.
Choose a hollow/ tube spindle with punch thru tightener. While it cost more, you will not have loose/ tercabut problem.
From outside looks the same. It’s the inside.
